- Refer to your employee EAP Wallet card for this code or contact the HR office at 469-4070.

- Employees and other family members living in the same household have available up to 5 free visits per problem, regardless of the number of family members involved or associated with the problem or issue. If the EAP provider is unable to resolve your (or your family members') problem or issue within the 5 free visits, OR if the problem requires that you see a specialist, there is a cost associated with the visits to the specialist. Health insurance may pay a portion of this cost according to the provisions of the health insurance plan. Coverage may only be provided for dependents who are eligible or covered by the employee's health insurance.
