  • Age and amount of coverage determine rates.

  • Cost for spouse coverage varies with age.
  • $2,500, $5,000, $7,500, or $10,000 voluntary insurance is available for each qualified dependent child.
  • $10,000 - $500,000 voluntary insurance is available for spouses.
