Does an employee receive an identification card?

• Yes - All new employees are provided with a vision plan ID card. Replacement cards can be obtained by calling Community Eye Care customer service at 1-888-254-4290.

May I purchase my vision hardware from somewhere other than where I have an eye exam?

• Yes. However, if you choose to purchase your hardware from an Out-of-network provider, your reimbursement will be lower. Please refer to the Out-of-network benefits summary or contact Community Eye Care customer service at 1-888-254-4290

 What do I do if I need safety glasses?

• Any employee who needs safety glasses will go through Gail O'Connell, Employee Safety Coordinator and/or Rich Jardine, Safety Technician
• The safety glasses will be purchased from Correction Enterprises (an enterprise that provides jobs for minimum security NC Dept of Correction inmates)
• The employee will be provided with the instructions on placing the order, fitting at EyeCare Center in Cary, etc.
